FWCS Payroll Department requires a minimum $18 gift ($1 per pay period) in order to process via payroll deduction. In order to facilitate payroll processing, please use your FWCS email address when submitting this form.

If making a payroll deduction gift that is not listed as an option, please click on the “Other” button so that it shows dark blue, and then enter your desired TOTAL gift amount. That amount will then be divided by 18 to determine your recurring payroll deduction amount.
