City of Fort Wayne Giving

Participate in the City of Fort Wayne Employee Giving campaign from March 14 – April 14, 2022.

By supporting Arts United, you’re helping make arts and culture available every day, everywhere, for everyone in northeast Indiana!

Instructions and other information:

  • The City of Fort Wayne requires a minimum $26 gift ($1 per pay period) to process via payroll deduction.
  • If making a one-time cash/check gift or would like to be invoiced (Bill Me Later) at a later date, please choose “Other” as your payment method. You will then need to forward your gift confirmation email to your City HR representative along with your cash/check.
  • If making a payroll deduction gift that is not listed as an option, please choose “Other” and enter your total gift. Ex: $39.00 ($1.50 x 26 pay periods).
  • Your email address will only be used for gift confirmation upon submission of this form.
  • Your home address will be used for the purposes of sending tax documentation and quarterly newsletters about the work Arts United is doing in our community. You will not be solicited for gifts at the address provided while participating in a workplace campaign.
